As of April 2022, they were booking late summer appointments for children and fall appointments for adults. No referral needed for child evaluations. Referral from health care provider for adult evaluations can be faxed to (920) 926-8933. In addition to private insurance, they take all forms of Badgercare except Trilogy, MyChoice, and Molina.
